Change Text Field into a Drop Down Box

In Design view of the form, right-click your text box and choose Change
To...Combobox.

You will get some new properties. One of them is RowSourceType. Make sure
this is Table/Query. Another property is RowSource. When you click in that
property, you will find a Builder Button [...]. Click that and create a
query that returns the records you want.

To remove duplicates, you can either switch to SQL View and add the word
DISTINCT after SELECT (SELECT DISTINCT * from ...) OR right-click in the
Table area of the Query Builder, choose Properties and set the UniqueValues
property to Yes.
