You can't have an *image* plus a number. But you can have a *box* plus a
number. Just find a box that's a character.
You can set up numbering to do "Chapter 1", "Chapter 2". So, you can also do
"[box] [number] [any 'text' you like]". The only proviso is that everything
in the "numbering" has to be the same font and the same font size.
Here's one way to try. Within your document, use Insert > Symbol. Try Arial
font. Assuming you have a recent version of Word, Arial includes lots of
(unicode) symbols. Look in the Geometric Shapes group of symbols. Insert
into your document the box you want in the "numbering". Select it and copy
it.

That page talks about the built-in Heading styles, but you can use other
styles if you choose. The same principles apply. When you get to the
Customize Outline Numbered List box, delete everything in the Number Format
box. Click Font and choose Arial, then OK. Click within the Number Format
box and do ctrl-v to paste your box. Then add additional elements to the
Number Format box (including your number) as described on that web page.
