I think the First Letter tab is for words following those
on the list (so you can put those abbreviations in a
sentence, without the next word being capitalized if you
have checked capitalize first letter of sentences).

Look at your "Replace text as you type" list. You may
find the offending words are there. I do not believe that
the "Exceptions" button applies to that list.
 
The operative correction here is probably the "first letter of a sentence."
FWIW, I've never seen "inc." used as an abbreviation for "enclosure."
