Audit Trail

I need to create an Audit Trail based on changes in values from a form. I
have an audit table and can populate an entire record into it but I need to
be able to audit and the field level, so I am only tracking an individual
field that changes, i.e. fieldpreviousvalue, fieldcurrentvalue, fieldname,
currentuser(), dateofchange.
Can any one help?
