How can I associate an icon with any file, like you used to be
able to do with Win98. You used to be able to right click a
file and associate an icon with it. I am only able to do this
with folders in XP.

Win98 did not have that feature unless dealing with DOS programs and
exactly the same applies to WinXP as well when creating .pif files.

Create a shortcut to that file, then you can associate any icon.

One particular file, or all files with one extension?
You can right click, select properties, and select change icon if it is a
shortcut.
For an entire extension, open Windows Explorer, select Tools, Folder
Options, File Types, Advanced.
