array index out of bounds

my problem is someone received an email with an attachment
of a custom form. when the email is opened, it returns the
error, "Can't open this item. Array Index is out of
bounds." does anyone know what could cause this?

Clearing the forms cache usually fixes it. Choose Tools | Options | Other |
Advanced Options | Custom Forms, then click Manage Forms, then Clear Cache.
If simply clearing the cache doesn't fix the issue, try the other steps
