Are there columnar pad formats for printing your own?

I've been using a columnar pad (5 columns) to do some accounting
stuff, and just wondered if there is a way to enter your own
headings and entries in a form, and have the inkjet print it in
columnar pad format. In other words, the background would look like
columnar format, with all the little shaded boxes.
