Adding a Logo

I want to add a logo to my signature. I was able to create custom stationary with a logo. It worked fine for a couple of weeks then one day it just turned into the dreaded "red x" I created a new one. Same thing. Now I can't get it to work at all. Any suggestions?
 
Make sure the picture is in your signature folder as well.

If you move the pic or delete it then it will not show up on the stationery.
As stated above, the picture must be in the same folder with your
stationery.
