Windows XP creating an admin account

is there a way to do this with a boot disk? I imagine that it is possible, but I have yet to find such a tool. anyone know how?
also, does anyone know a way to manually enter a hash for a user to restore a forgotten password?
 
Is this computer on a network or is it a personal computer that you have forgotten the password for..? What OS are you using...?
 
A program called n-lite will sort the first part of your question.
