Word 2003 Index

I have created an index for a large document. It's formatted to 2 columns.
My problem is with the text wrapping. It's breaking up items that should be
grouped. For example, I have a header of "Exam Objects." There are 7 words
associated with this header. 2 of those words are at the top of the next
column with nothing to let the reader know that they belong to Exam Objects.
I have manually added "Exam Objects cont'd" at the top of the column, but
when I refresh, it disappears. I have tried changing the paragraph
properties to keep them together, but they reset upon each refresh, too.

Does anyone know of a way to keep these items together.?
 
Unfortunately, there is no satisfactory way to do this. You could format the
Index 2 style as "Keep with next," but you really only want this formatting
for all but the last subentry, and you need it for the Index 1 style, too.
What I have done in such situations is wait till editing is complete and I
am sure the pagination will not change, generate the index one last time,
unlink it, and apply manual formatting as needed.
