volume icon doesn't always appear in taskbar

I've customized my Taskbar Properties to "Always Show" the volume icon, but sometimes it doesn't appear on start-up and the Taskbar Properties customize section lists it as a "past item". Usually it will reappear if I restart. What's wrong?
 
Try rt clicking on Task Bar and selecting properties. Then uncheck Hide
Inactive Icons.
