Trojan?

Is this a trojan and if so how do i remove it: "downloader.Mediket.A"

I've scanned with anti-virus, spybot, adaware and cw shredder and it keeps
coming up clean. I also turned the system restore on and off and still this
thing remains on my computer!

update your virus definitions and try again?

A lot of the hits found by google are russian - perhaps this is a new one
that hasn't spread far yet?

Make sure you do the scans in safe mode after updating the definition
files. Also you might want to post a HiJackThis log to one of the
specialty forums, _NOT_ this one.
