Table of contents not updating properly

I am trying to update my table of contents that is using heading 1-4 as the
basis for the entries. When I update the entire table, 75% of the table of
contents disappears and some of the heading 4 entries remain in the table of
contents and one heading 3 remains (there are a lot of others). The styles
are still there in the document. I'm not sure what to do. Can anyone help?

My guess is that the entries in the TOC were actually typed in. Browse in
the body of your document to locate some of the text that has gone missing
out of the TOC and make sure it is formatted with a Heading Style or a
Outline Level. Chances are pretty good that once you fix the text, it will
appear in the TOC again.
