Simple Photo + Image Annotation and Markup Tool or Method in (an existing tool)

I've done an exhaustive freeware/shareware/commercial search and have
yet to find a simple application to mark up and annotate a photo or an
image in its native format (JPG or other).

I just want to be able to hilite certain areas in the photo/image by
drawing a bright circle or ellipse around it, point to it using arrows,
add a short text description at the tail of the arrow and save it back
in the format it started with. A crop function may be nice as well.

I did look closely through alt.comp.freeware and there are many ways I
can do this with Powerpoint, Openoffice... but one must import and
export the JPG.

Seems like this is not out out of the ordinary in terms of need but
there is just nothing that I can find that'll do the job. Any ideas?

Output in a wide variety of formats inc. several graphics, as well as
Flash, HTML and .EXE. Freeware.

PC Magazine also have a utility for this; can't remember the name, but
it's newly-updated. You need to pay some version of their fee to download
it.
