Implementing an approval workflow for delegating administration

I'm looking for a solution to provide an approval process and audit
tracking for a delegated OU.

The scenario is:
- client submits request for access change.
- their manager approves
- application owner approves
- system implements the approved changes and logs who approved, what
change, and when.

Anyone know of a product or preferably open source project providing
this functionality?
