How to create the Add Connection Dialog using VS2005?

I want to provide user a connection dialog similar to or same as the Add
Connection Dialog including Advanced Properties window.

I see something called DDEX but it seems to be geared for third-party add-in.
I have tried to follow the section Implementing the connection control in
http://msdn2.microsoft.com/en-us/library/ms379576(vs.80).aspx#ddexintro_topic2a
but I don't see the connection dialog at Step 7.
