defrag doesn't defrag

I'm using Windows XP Home Edition. My drive is partitioned into 3 drives. I
want to defrag my C drive which has Windows on it. It's NTFS format. It has
about 25% free space. I defragged it once (upon recommendation from the
program to defrag), but when it finished it's long process, the files were
still fragmented and there were some errors saying that some files couldn't
be defragged. So, I started the computer in Safe Mode and re-did the process
only to get the same results.

What's the problem? What should I do?

Some files are not defragmented by design. If you want the technical details

Well, besides the files that were not defragmented, overall after I
defragmented the hard drive, the defragment analyzer still reports that my
hard drive needs to be defragmented. If a few files are not defragmented, I
suppose that's not a big deal, but if after defragmenting the program still
shows that I need to defragment, I guess there's a problem.
