In which folder are the Start menu items?

Which folder/s contains the Start mentu items?

Some of the menu items are user defined and so are based on recent
use (eg. Word). Other items are fixed (eg. My Documents).

Where are these kept or defined?

Typically:
C:\Documents and Settings\All Users\Start Menu
and
C:\Documents and Settings\{username}\Start Menu

Chris pointed you to where items found under 'All Programs' are stored.

In addition:
Recently-used documents are stored here:
"C:\Documents and Settings\<username>\Recent" (display name: "My Recent Documents")

Items pinned to the start menu are in the registry under this key:
"HKEY_LOCAL_USER\Software\Microsoft\Windows\CurrentVersion\Explorer\StartPage"
(the 3 'favorites' values)

Frequently used program info is stored here:
"HKCU\Software\Microsoft\Windows\CurrentVersion\Explorer\UserAssist"

Is it possible to increase the number of recently-used documents
which are displayed?

Can the document names be displayed in a different order? For
example, by recent usage, or by size, or whatever.

Not to my knowledge...not sure if it's related to history settings or not.
