Very Strange - Any ideas?

I have a Custom Contact Form which I have made my default within my personal
contacts folder.
When I do New Contact, it pops open the form ok, but auto-populates it with
an old contact's details that doesn't even exist in my folder.
I have gone into the Custom form and cleared any fields that had data in
them, such as Name, and Address etc. Re published the form.

However, when I open the form via Tools, Form, Design Form and choose the
published form in my Personal Forms Library, the Name and Address field is
populated again.

How do I save this form so it created a new blank form??

Please help. Many thanks.

Managed to fix this. The General sheet tab although not being displayed had
some fields populated with data. This is where the custom sheet was pulling
its info from.

Deleted the data from General fields and all is well.
