BCM Appt Plus makes Outlook crash

Whenever I click on one of the buttons next to "I'll Call Them At This
Number" etc.,
I immediately get the error message saying :Outlook has stopped working" and
"Outlook is Restarting." I am using Windows Vista.

Does anybody know a fix for this?
