Try removing the space from the page file name
Rename it to Apply-Online.htm. The web host may offer another solution, but you have to check with them to see what is offered or

I just spoke to the host, he said that he has a Unix based server and he is
unable to enable email transport. He said that I would need to go onto a Microsoft based server. Is this correct?
