Try removing the space from the page file name
Rename it to Apply-Online.htm.
Check w/ your host that they have email transport enabled

Optional: The web host must configure the email component to work with each web site using the FP
extensions and Form Handler.

The web host may offer another solution, but you have to check with them to see what is offered or
supported.

I just spoke to the host, he said that he has a Unix based server and he is
unable to enable email transport. He said that I would need to go onto a
Microsoft based server. Is this correct?
