Auto Notes

I am a web developer on a technical support site. I'm trying to create a page
that as you answer questions on the page (click yes, click no, etc) it
automatically generates "notes" in another window (notepad or empty text
field) corrisponding to the option you selected. Can someone please help?

probably a layer or popped-up window, but what would the notes contain, and
where would that information actually come from?
