Default Forms opening blank


I have a form in our organizational library that opens fine except for 3
machines in which the forms open as a blank e-mail. They are all on Outlook
2003 sp2. Any ideas as to what may be causing this?

I verified that this was not checked and we still have this issue.

It seems that Office SP 2 caused this, have you seen this before?

For those users, what does the Help | About This Form dialog tell you about the form version? Are they seeing the latest? If not, have each user try to create a new item with that form.
