Send - Receive Button fails to appear in Outlook 2003

I have installed Outlook 2003 and used the email setup wizard and still the
Send - Receive button does not appear. I have uninstalled and reinstalled all
to no avail. Can anyone help with a solution?

Either reset your toolbar (customize, toolbars, reset) or close Outlook,
rename outcmd.dat to .old.

Dear Milly,
Thank you for our response. I have reset the toolbars with no success. I
have renamed the file as suggested and this produced an error which forced
the program to close and send an error report to microsoft. On restarting
there was no send receive button. I deleted all the mail accounts and
re-entered them using the wizard and still no send receive button. I would be
grateful for any further suggestions to try.
