I published duplicate form and can't delete it

Default I published duplicate form and can't delete it


The form doesn't appear in my folder list, but when I click on "choose form", the dupe appears in the list of forms in the folder
with my original form. The delete option is grayed out. Can you tell me how to get rid of the extra unwanted form?
