Default Faulty send on behalf

A meeting request was sent to several users by a meeting organizer (we'll
call her A) and one of the recipients (we'll call him B) replied to it with a
question. His reply was a reply - not an acceptance of the invitation. When
the reply was received by the meeting organizer (A), it said "B on behalf of
A" in the From field. I don't understand how this happened because B is not
set up as a delegate, nor is he set up with "send on behalf of" permissions.
Has anyone ever seen this?
