Default creating new fields

I have been searching for some time now, if anyone can
help, that would be greatly appreciated. I want to create
a new field in Contacts to have a box checking option. I
have a list of attributes that would best be expressed
with a list and boxes empty or checked next to them. Is
this possible? How would I go about doing it?

Thank you for your help!!

It's called a custom form. See http://www.slipstick.com/dev/forms.htm
