Default educational forms

I installed Microsoft's educational form for Outlook
(Technical Repair Form). It is located in the
Organizational Forms Library. Every computer across my
network finds the form and the drop down fields from the
central Access Database. My computer is the only one
which does not find the drop down fields from the mdb
file. The form loads with no problems, but the drop down
fields are empty. I am using Outlook 2003 and all other
computers are Outlook 2000 or earlier. Could it have
something to do with Outlook Security settings? Please
help.

Did you follow the installation instructions at
http://www.slipstick.com/addins/mssampleapps.htm and publish the form?
