Default Converting an Excel form to an Outlook form


My manager has a fill in Excel form with lines and check boxes and he now
wants this converted to a form in Outlook in the form of an e-mail. This is
totally new to me and I don't really even know where to start. Do I
basically have to rebuild this Excel form?

Yes, that's exactly what you have to do. See

The first question to ask is why he wants it to be an Outlook message form
and whether the recipient has Outlook and will have access to the published
form definition.
