Created a Form in office 2k - details disappearing

Default Created a Form in office 2k - details disappearing



Can anyone help ive created a form in outlook 2000 and then published it
but whenever i enter details on the second page and then send it, the
recipient recieves this ok but all the fields on the second page are
blank.

Is there an option that i have to change so that the values on the 2nd
page remain ??

Im sure this must be something really simple but i cant find the
answer. Any ideas welcome.
