Default Online Forms


Where could I get a FrontPage 2002 template that collects First Name, Middle
Initial, Last Name, Street Address, City, State, Zip Code, Phone
Number,(optional,) and E-Mail Address, and stores them in a text file or
adds them to a web page?

Dick,

You really don't need a template for this, as FP offers a form wizard that
will do everything you are asking.

You also could create your form manually and then "Right Click" within the
form and select Form Properties. Here you could send to a text file.

NOTE: FP forms require FP server extensions

Thanks, JDR! I really appreciate your feedback. I had tried the Form
Wizard, though I didn't entirely understand how best to use it. This makes
it much clearer.

Default Follow-up Question for JDR re Online Forms

I just noticed that the Microsoft web page below pertains to FrontPage 2003.
However, I'm running FP 2002. Would the same instructions apply? What, if
any differences might I encounter?

it also *does* have a template - a basic contact form - under File > New > Page,
then choose template from the panel on the right side of the screen that should
open.

Choose "page template" then choose "Form Page Template" from the dialogue.

You can add extra fields to the form, but it gives you the basic idea.
