strange error: IRP_MN_QUERY_INTERFACE for default process

I capture the occasional error with Windbg.

My driver is a BDA driver + usb driver, when I pass system irp
IRP_MJ_PNP: IRP_MN_QUERY_INTERFACE to lower layer,
this error happen:

Help!

My wrong case:

USBDriverEntry begins
