Sounds as if the operating system is corrupted somewhere.
Next thing I'd try is create an entirely new power user account,
log on to that, and see whether the defect is there as well.
If so, and unless anybody else comes forward with a better idea,
you could try to reinstall Windows XP as an update, then also
reinstall the latest service pack (I think it's still #1). This
should, at first, keep your installed software and settings.
After that you may still have to recreate the user account.
