I have the same problem as Geert, but I'm using Outlook 2002 SP3.
Unfortunately, following the steps you recommend does not work for me.
I was sent an OFT template by another designer who had his signature
embedded in the form. I have tried opening a new item and removing the
signature, incrementing the version number (there was none on the
original file) and publishing to my Inbox. I have repeated the process
a few times to no avail.

In that case, the only solution I know is to use Mdbvu32.exe or Outlook Spy
to delete what is apparently a hidden attachment (or to write code to do the
same).
