Modem UNINSTALLS after installing 9600 256MB ?

I just bought a Radeon 9600 256MB card and having this very strange
problem. The problem is when I boot my computer more than half the
time the modem gets uninstalled. The computer just started doing this
when I plugged in the new 9600 card. When I go to ADD REMOVE programs
it still can not find it. I do not get this with my OLD NVIDIA card.
It still does this even after I did a clean install of the Operating
System. I even changed the slot for the modem twice on the
motherboard and still no luck. Any clues on how to prevent this?

Computer Specs:
AMD 1.1 MHz
512 RAM
OS: ME
40 GB Hard Drive
ATI Radeon 9600 256MB (3.8 Driver)

This is mostly a guess, but maybe the modem and video card are sharing an
IRQ signal or bus or something. Try plugging the modem into another slot.

As stated above in my orginal post I have already tried that. Any
other ideas or anyone else have this same problem?
