I need to have a form that will allow the viewer to insert their e-mail
address and click submit to be added to a mailing list. I used one from FP
which gave room to type and then a submit button, however, when going to
preview or view in browser it did not allow entries.

The FP Form handler can only send email and/or write to a file or store info in a database via
ASP/Access or MS SQL.

You must test the form on a server that has the FP extensions via http://
