form page won't work

I am having a problem getting my form page to work. It might be due to me
cancelling the fp extensions since I've installed some scripts that need for
me to ftp my changes, so I won't overwrite the files my client has updated.
I needed to add a forms page to their site, but I get this error when I try
to reinstall the extensions.
Can anybody tell me what I should do now?

FP forms need the extensions... ftp'ing breaks the extensions.

I tried to reinstall, but not having any luck.

Reinstall what?

Letting the client upload stuff is usually a disaster and lots of extra work
for the webmaster. I usually charge double for fixing broken stuff.

No, I didn't mean that. I have a PHP script installed where there are
places that the client can fill in. I'm just afraid if I publish, I'll over
write what they have done. I also have a calendar script installed and
don't want to over write that with my files.
Any ideas?
