i have a dwt setup with include pages. top, left, right,
and bottom. i would like to keep this template throughout
the site, but i would like to change the include pages
for some specific pages, for example the top header
include to have different ads on it. Is this the best way
to handle this?

No

First of all using Include pages in DWT's is unnecessary and not recommended
(and will limit use of some features in FP like Interactive buttons, behaviors, etc)

Create the DWT w/o the includes (included content your DWT page) and apply it to your other pages
Make a copy of the DWT and modify it for your different pages and apply the DWT to the different pages

Unfortunately FP does not support nested DWTs so you have to maintain 2 DWT templates

Ya.. I was asking this question a while back.. I thought that it could support nested templates. Basically I read this statement in Microsoft Office FrontPage 2003 saying.

"One Dynamic Template can attach another. In this arrangement, changing the first template updates the one that attaches it, and this in turn updates any pages that use the second template.

This would be really useful to me as well. I can't seem to get it working either though so I'm guessing that FP 2003 don't do it
