Try replacing the floppy drive. I've seen this a number
of times before--I think it's called the "change line"
signal. One of the failures I've seen is where the floppy
drive itself does not realize the disk has been changed.
A side effect of this is that it can write root directory
information from one disk onto another, thus corrupting
the second disk.
