I would like to create forms and slips for clients to reply to me with a
simple "tick box" for them to tick. Despite this being a very common need
the tick-box does not appear in any character set that I have seen. Does
anyone know how to create them please?

Thanks

Andrew

Take a look at how it is done with the fax template that comes with Word, or
take a look at the following series of articles:

better records!
