I was wandering, how do make a company clearance form for our employees who
request for a copy of their clearance certificate from previous employer.

First, stop wandering and sit down while you're on the computer.

Since I've never heard of a clearance certificate, maybe you should search
Google for help. And what does this certificate clear for a former employee?

If you are referring to a clearance certificate required by any
government agency, e.g. income tax agency such as Revenue Canada, there
is probably information about that, possibly even a sample form as a PDF
file, on that entity's web site.

John Jacob Niles, the singer and collector of folk songs, said that he
based his "I Wonder As I Wander" on a line or two of haunting music that
he heard sung by a young girl in a small North Carolina town. He asked
