Toolbar customizations are stored in outcmd.dat but an addin that creates
its own toolbars and then deletes them won't dirty that file. To get rid of
something like that the only way is to uninstall the addin and then if
outcmd.dat is left dirty the customize UI can be used to delete any added
toolbars and to reset any default ones. Deleting outcmd.dat is also possible
but can be like swatting a fly with a sledge hammer, it will kill all
customizations.
