How do I keep Word 2003 from loosing custom toolbar commands?

Custom toolbar commands disappear when I close and restart Word. This seems
to be a common problem but I have not found a solution.

where and exactly how did you save and assign the custom command? What
is it supposed to do, run a built-in command, start a macro, etc.?
