An antispam scanner on one of the mail servers is adding [spam] to the
subject - it could be your server or the recipients.
Version of Outlook? If Outlook 2007, the recently released SP2 will add
information to the Infobar telling you if it moved the message. It won't say
what other program did it, only if it was it's own junk mail rules or
something else.
What type of email account do you use?
Does it happen if you start outlook in safe mode?
