How do I fix shell32.dll icons?

Hello:
I want to customize a folder with an icon from shell32.dll, but there are
some "blank" spaces (blank icons) in the dialog that shows them. My version
of shell32.dll is 6.00.2900.3051 (xpsp_sp2_gdr.061219-0316). For example,
icon index 162 is blank (A resource editor shows this icon is a 16x16 blank
icon)
I tried TweakUI Power toy but didn't fix it.
