I would like to create an electronic pro-forma for a copy shop ordering
system. Typical fields will be: name of person requesting, quantity, single
or double-sided, cover sheet with choice of colour, whether stapled and if
so, top left or left edge.

The pro-forma will not be sent unless a valid sender's name and quantity are
entered.

The form should pop up when a particular icon is selected as a printer driver.

You didn't say anything about the Outlook version, who'd be using this, or
how it would be used, and I definitely don't understand the connection with
a printer driver.
