How do I automate a simple email with info coming from a form?

I'm trying to set up a simple email system whereby I can click on a
hyperlink, or command button, on a form, and the email addresses, subject
line, and a small message will appear. I know that the email addresses and
subject can be done simply with the hyperlink options. I just want to have a
one line message with one or two areas drawn from fields in the form. Any
suggestions? Thanks!
