DMS formatting

I am working on direct deposit payroll receipts and am having problems with
formatting.
For some reason one of my book marks is printing up in the header
information even though I clearly see it in the body of the page.
Plus I would like to have better number formating for $ amount. If I could
at least get the ',' in for the 1,000 I would be very happy.

Any help - please.
