I understand how to use a SQL database for the BCM from the document but I
would like to set customer fields and form with reduced information to make
it clearer to the user without going to each station and downsizing the form
for each user. Is there a way to do the form once and then have everyone use
that look and feel.

BCM custom fields are per database, so all shared users will see the
same fields.

BCM doesn't support any other form customization.
