If I open my scanner in Control Panel, there are three device events
corresponding to the three buttons. With Office XP, there was a "send to
application" entry for "Microsoft Office Document Scanning" which could
be assigned to an event, with Office 2002 all I have is an entry for
Photoshop.

Anyone know how to add applications to this send to list?

You can add apps to the send-to list by going to "Document Settings-><your
login>->Send To" and create a new shortcut to the app. For instance, to add
Notepad to the send-to list -- go to the "...Send To" directory, right-mouse
New->Shortcut, browse to "C:\Windows" and select notepad.exe, "Next" button,
and give it a name.

In your case, you probably need to specify an "event" on the command line,
too. If you can look at the properties of Office XP shortcuts, they will
show you what you need add.
